Purchase Authorization: Complete this form as soon as you obtain the charter cost. Submit form to the appropriate parties for approval. Email a copy of the approved form to the Trustees (trustees@fcbc.org) for bus verification. If paying with church credit card, Trustees will make payment. All costs are to be covered from your ministry/department budget. If paying by check, submit a check request.
Prepare a list describing equipment/supplies to be transported and dimensions for the bus company.
Facilities/Activities Request: Complete this form to record your activity on the church calendar.

Notes:

If the driver stays overnight, the bus company may require your ministry/department to provide a hotel room.
If the driver stays overnight to continue the trip, the suggested tip is $1/day/rider.
Cancellation policies vary. If a bus is no longer needed, contact the bus company ASAP.
